The Area Agency on Aging Region 9, Inc. (AAA9), located in Cambridge OH, is interviewing for: ADRN Assistant/Intake Assistant ADRN Assistant - Perform general clerical duties, handle requests for information and data. Prepare correspondence, schedule meetings and appointments. Will also work as an Intake Assistant ? Collecting information and identifying needs about an individual?s long-term service and support needs. Will capture and record responses in the state system. Applicants must have a high school diploma, Associates Degree in a related field preferred. A minimum of three years of experience in a high volume medical or long term care setting serving aged or disabled populations required. Must have excellent computer, communication (verbal & written), clerical and telephone skills. Must be customer service oriented and have excellent people skills. Case Assistant-SRS Program Provide client related clerical support to the Consumer Care Specialized Recovery Services Division. Generate, copy, process, log, track and provide follow-up as needed. Applicants must have a high school diploma and one year of experience in a related position. Must have excellent computer and telephone skills. Must be able to effectively use Microsoft Office software including Word, Excel and Outlook. Experience in a medical or Medicaid/Medicare facility preferred. Home Choice Transition Coordinator Provide pre-transition services to Home Choice participants which may include but not limited to: housing assistance, coordination of benefits, establishing budgets and many other activities to assist participants with their discharge from an institutional setting. Assists participant with pre-discharge planning, discharge process and placement in the community for 90 days after discharge. Will have in person and telephone contact with the participant and/or guardian. Applicants must have a minimum of 1 year of experience delivering services in a health care, behavioral health or social services setting or completion of a bachelor?s degree in social or behavioral science, health services or comparable field. Experience advocating on behalf of individuals with chronic illness, mental health and or/substance use disorders, physical and/or developmental disabilities. Must have a minimum of a high school diploma or equivalent and have a valid driver?s license and proof of active automobile insurance.
